ORDERING INSTRUCTIONS: Shipping for each Perma Plaque® item is $8.50 for each plaque and $11.50 for each Gift box with some exceptions. All Plaques shipping to a California address must include 8.25% sales tax. Plaques may take eight (8) weeks or more to ship after receipt, boxes may take ten (10) or more weeks to ship after receipt. All orders must be pre-paid. Before sending, please remember to protect your documents between stiff cardboard or other protective shipping material and mark the package DO NOT BEND clearly. With your document(s), please include your check payable to Perma Plaque by ICU, e-mail address, phone number, shipping address and color choice(s).

We do not accept credit cards. Checks and money orders only.

  • Parcel Post Shipping to P.O. Boxes may delay shipping (surface): $12.00 each Plaque, $17.00 each Box
  • There is an additional $8.00 charge for each additional piece on the same Plaque or box.
    Examples are a diploma and a picture on one plaque or the inside of an invitation on the inside lid of a box.
  • Plaques over 375 sq. inches shipping $25.00 (UPS), $33.00 (parcel post).
  • Note for box orders: Please refer to specific box color choices on the home page or on the colors page (bottom) as well as box lining color choices. For colors not specifically listed for boxes, you may choose from any of the plaque colors for a custom color charge of $8.00.
  • The default trim color is gold (border and bevel) but you may specify silver or none if you choose.
  • Some materials will not adhere to the acrylic coating and some inks are heat sensitive. In these cases, a solid piece of acrylic is attached with screws at an additional cost of $12.50. In cases where the type of ink is not evident, we cannot take responsibility for the integrity of the document.
  • Please include a check with your order, we do not accept credit cards.
  • Please use the order form.
  • We are not be responsible for lost or damaged items.
